A. Key Dates:
1. Online Form Submission Begins on 30th Jestha, 2081 at 10 AM
2. Online Form Submission Ends on 18th Asadh, 2081 at 5 PM
3. Late Fine Online Form Submission period: 18th Asadh, 2081 at 5 PM – 25th Asadh, 2081 at 5 PM
B. Entrance Exam Dates:
1. MBBS: 1st Bhadra, 2081 (11:00 Am to 2:00 Pm)
2. BDS: 2nd Bhadra, 2081 (11:00 Am to 2:00 Pm)
3. BSc Nursing, BASLP, B Perfusion Technology: 3rd Bhadra, 2081 (11:00 Am to 2:00 Pm)
4. BAMS. BNS. BS MLT, BSc MIT, B Pharm, B Optometry, BPT: 4th Bhadra, 2081 (11:00 Am to 2:00 Pm)
5. BPH: 5th Bhadra, 2081 (11:00 Am to 2:00 Pm)
C. Required Documents:
1. Citizenship Certificate
2. National Identity Card / NID Application Verification Paper (राष्ट्रिय परिचयपत्र सम्बन्धी आवेदन रुजु फारम)
2. Recent pp size photo-1
3. Bank voucher / Online Payment Receipt
(Different Vouchers for Different Programs)
4. SEE Grade sheet or equivalent
5. Transcript of +2 or equivalent
6. Admit card of Grade 12 in place of +2 Transcript for Grade 12 Board Exam result awaiting students
7. Recommendation letter/s for reservation quota
8. License from the concerned council (if applicable)
Note:
1. The date on the voucher should be in B.S
2. If a student has passed classes 6-10 from a government school then she/he should get a recommendation letter from the school and education wing of the VDC / Municipality office.
